Auto Insurance + Massage
If you were involved in a Motor Vehicle Accident (MVA) in the State of Oregon, your medical expenses are covered up to at least $15,000 or more. PIP (Personal Injury Protection) insurance is part of every insurance policy written in the State of Oregon. It applies whether you, or another driver were at fault, or if you were a pedestrian or on your bike. This benefit is inclusive of massage, chiropractic, and acupuncture, as well as medical care, prescriptions, and lost wages related to your injury.
It is your legal right to choose the practitioner you want to work with.
When you come to your first appointment, please bring:
- Prescription/Referral with diagnostic code(s) from Attending Physician (DC, DO, ND, MD, etc)
- Insurance company information
- Adjuster’s name and phone number
- Incident claim number
- Date of accident

Epic Wellness NW will directly bill any auto insurance company (for example: State Farm, Progressive, Allstate, Nationwide, Country, Geico, etc) for massage therapy. There are no co-pays associated with MVA billing for massage therapy.
Worker’s Compensation + Massage
If you have been injured on the job, some workers’ compensation insurance plans will cover massage therapy treatment. Contact the medical doctor that is treating you for your worker’s comp claim to get the prescription for manual therapy so that it will be included as a part of your treatment plan. Contact the claims adjuster to inform them of your doctor’s prescription for manual therapy and make sure it will be covered.
When you come to your first appointment, please bring:

- Insurance company name, address and phone number.
- Adjuster’s name and phone number
- Incident claim number
- Date of accident

Health Insurance + Massage
Many health insurance policies will cover the cost of massage therapy. Health insurance plans vary in their cost coverage of massage therapy and the conditions under which the massage therapy services are received. Certain health insurance plans require a doctor’s referral in order to cover the cost of massage therapy while other health plans allow a certain number of visits without a referral. Some health insurance plans cover massage therapy costs in full, while other health plans require a client co-payment.
It is best to check with your health insurance provider for the exact details of your insurance policy. Our providers are qualified to bill for 97124 (therapeutic procedure) or 97140 (manual therapy techniques).

Massage therapy is often prescribed by physicians, naturopathic physicians, and chiropractors as part of a treatment plan for these and other medical conditions:
- recovery from surgeries
- tendonitis
- sprains (ligaments) or strains (muscles/tendons)
- osteoarthritis
- repetitive strain injuries
- athletic injuries
- sciatic pain
- fibromyalgia
- carpal tunnel syndrome
- thoracic outlet syndrome
- temporomandibular joint dysfunction (TMJ)
- swelling or edema
- insomnia
- anxiety
- emotional stress
These and other conditions can be effectively treated with medical massage, depending on your physician’s diagnosis and prescription.
If your health insurance plan requires a referral or prescription to cover the cost of massage therapy, please provide us with:
- Prescription/Referral with diagnostic code(s) from Attending Physician (DC, DO, ND, or MD)
- Some insurance plans require that frequency and duration of visits is specified on the prescription
- Your insurance card
